This review focuses on the complex interplay between genetics and potential modifiers of disease expression including demographic features, physiological challenges such as pregnancy and physical exercise, as well as comorbid conditions. Some of the potential modifiers (Table 1) will be used as examples to discuss gene–environment interaction in this particular clinical setting.
