Every day he has to choose and to act not necessarily absolutely the best but, pragmatically, the best for his individual patients, I believe, like many others authors [2,3,5], that EVH must be in the armamentarium of coronary artery surgeons and should be preferred over the OVH, by experienced operators, in selected patients for merely aesthetic motivations or for reducing leg complications’ risk in high-risk patients (as diabetic, obese and with peripheral vascular disease). For other patients, a vein with minimal conduit damage (OVH), probably better a “no-touched” vein, should be used to ensure its best patency durability and therefore the highest benefits for patients undergoing coronary surgery.
