Transaminases are intracellular enzymes released and found in the blood after cellular damage. Serum levels of transaminases are tested commonly in children and hypertransaminasemia is a common finding. Hypertransaminasemia could be isolated or associated with other signs and symptoms, it is often secondary to liver disease but it is important to remind that many diseases not involving the liver can cause elevation of transaminases. The diagnostic work up for hypertransaminasemia could be complex and it should start always from the medical history and clinical findings, in order to investigate all the possible common and uncommon causes. © 2011 Elsevier srl.
