The ULTIMATE trial is an important addition to thebody of evidence investigating the role of IVUS inoptimizing PCI. The investigators should be com-mended for accepting—and eventually overcoming—the challenge to prove superiority of IVUS in patientswith mainly (but not exclusively) complex CAD.Collectively, and particularly in view of the totality ofRCT evidence, there is no question that the use ofIVUS guidance to optimize PCI does improve patientprognosis. Against this background, there is no sci-entificjustification for the observed inertia in inte-grating an imaging-guided strategy more broadly inclinical practice.
