A 57-year-old male with hypertension and hyperlipidaemia presented with chest pain, elevated troponin, and diffuse ST-depression in the anterior leads. Coronary angiography showed flush occlusion of the mid-right coronary artery (RCA, Panel A) and collaterals to the distal RCA from a minimally diseased left coronary artery. After two unsuccessful attempts of recanalization, the patient was referred to our hospital 6 weeks after the acute presentation.
