OBJECTIVES The aim of this article is to report a case of marsupialization of peri coronal tissue of a low impacted wisdom tooth. This technique has been chosen as an alternative way of intervention to the surgical removal of impacted third molars, as an attempt to avoid the mor bidity related to the extraction. MATERIALS AND METHODS This case report is about a 45 years old woman, who came to the attention of the authors, pre senting an asymptomatic osteo lytic area related to her impacted wisdom tooth. The treatment cho sen was marsupialization. The fol low-up was as follows: the first or topantomography and dentascan were taken at T0, than T1 corresponds at 6 months, T2 corresponds at 12 months, T3 corresponds at 18 months. RESULTS During the follow-up a progressive deposition of new bone has been observed around the impacted tooth. CONCLUSIONS Marsupialization can be considered an alternative way of intervention to the extraction of impacted wisdom teeth. That is, since these two ways of treatment have a similar effectiveness, but the marsupialization has a lower risk of morbidity. CLINICAL SIGNIFICANCE The marsupialization of pericoronaric tissue can be a conservative and resolving treatment.
