Apremilast a small moclecule in use for the treatment of moderate-severe psoriasis induced a strong skin hyperpigmentation in a patient with preexisting solar lentigo. As PDE-4 plays a key role in melanogenesis, we suggest caution in the use of Apremist in patients with moles and/or hyperpigmented skin lesions.
