An 86-year-old woman presented a painless, fast-growing lesion on her right forearm firstly noticed two months before. The complicated diagnostic algorithm is reported. It retrieved a diagnosis of spindle (or sarcomatous) cell squamous cell carcinoma(sSCC), a rare variant of squamous cell carcinoma, which has been reported to account for 1% of squamous cell carcinoma. It has the potential for an aggressive disease course, largely dictated by tumor size and depth of invasion.
