The transjugular intrahepatic portosystemic shunt (TIPS) represents a key intervention for portal hypertension (PH) complications. However, the epidemiology of chronic liver disease has changed substantially, with diabetes, obesity, and metabolic dysfunction–associated steatotic liver disease (MASLD) now representing the leading etiologies [1]. In this evolving context, TIPS indications and outcomes must be carefully reassessed.
