We read with great interest the article titled “Comparative outcome study of gynecomastia surgery and gender-affirming mastectomy with 100% nipple preservation” in which the authors investigated the surgical procedure in male thorax recontouring, comparing subjects suffering from gynecomastia to patients in gender-affirming surgery programs. Both types of patients have overlapping needs and share similar aims regarding the surgical procedure.1 Because there is a high demand for male thorax recontouring procedures, we appreciate the effort in the authors' investigation. The preservation of the nipple-areolar complex (NAC), regarding the shape and the position in the thorax, is one of the principle aims of the surgical procedure; it is also important to avoid any color mismatch and minimize postoperative morbidity and scars. Nowadays, surgical efforts are also focusing on the preservation of NAC sensitivity, which is a reason why we find the authors' study particularly interesting We completely agree with the authors that NAC grafting is affected by color mismatch and lack of sensitivity; however, we would like to discuss some further points.
