ABSTRACT Infection is a dangerous complication of total hip replacement. The incidence of infection has been decreased in the last years. The optimal surgical treatment is still debated. Two main techniques are described in the literature: the One Stage and the Two Stage replacement. The first consist in a direct exchange revision using a new THA and antibiotic cement. The One Stage technique is rarely indicated today. The Two Stage replacement is based on two phases. In the first step, the infected THA is removed and an antibiotic impregnated cemented spacer is leaved in place for a minimum of six weeks. The second step consist in a new THA. In the present study the Authors reported the results of Two Stage technique in treatment of twenty-three patients with an infected THA (nineteen women and four men). The diagnosis of infection was based on clinical, serological investigation (ESR, CRP), diagnostic imaging (X-ray, bone scan) and microbiological analysis. At a mean follow up of 5.8 years the Authors observed eradication of infection in all cases.
