Objective. We evaluate the possibility of an echotomographic differential diagnosis between Leydig Cell Tumors and Germinal Cell Tumors of the testis. Patients and methods. Since 1989, we have evaluated 6 patients with Leydig Cell Tumors of the testis. Patients were between 28 and 45 years old. In 3 asymptomatic patients the tumor was an incidental finding. Two patients presented gynecomastia. In 1 patient, the tumor was discovered by echotomography, performed for an acute orchiepidydimitis. The echotomographic examination confirmed the inflammatory disease and also detected a 6 mm focal lesion. All patients underwent an echotomographic examination and a color doppler evaluation. Two patients with a lesion over 5 mm underwent a retroperitoneal CT scanning to evaluate linphonode involvement: the result was negative. Results. The ultrasound aspect of the tumor was the same in all cases: a 3-18 mm ipoechoic focal spherical policycic lesion, with regular limits. The patients affected by acute orchiepidydimitis showed a focal lesion which was isoechoic with the non-omogeneous contiguous parenchyma. We have never found signs of abnormal vascularization with the color doppler imaging. A mass effect on the testis vascularization was detected in the biggest tumors. All patients underwent a conservative surgical therapy which confirmed the echotomographic diagnosis. Follow up ranged between 5 and 6 years. During the follow up none of the patients showed either local relapses or metastatic lesions. Conclusions. The Authors believe that the echotomographic and the color doppler imaging aspect, associated with clinic manifestations, are pathognomonic findings for the diagnosis of Leydig Cell Tumors and they advise conservative surgical therapy, on the basis of their results and those published in recent literature.
