Objective. Optimization of surgical therapy in La Peyronie disease (IPP) associated with recurvatum penis, according to plaque' s localization and associated erectile dysfunction. Patients and methods. Since January 1996, 12 patients between 45 and 63 years of age (median age 44) with IPP, have udergone surgical treatment in our Urologic Clinic. All patients had a more than 30 degree recurvatum penis and only two had erectile dysfunction. All patients underwent NPT test, a dynamic echo color doppler exam and power doppler exam alter intracavernous administration of PGE1 10 mcg. Those patients with erectile dysfunction underwent intracavernous administration of PGE1 20 mcg. Three patients with dorsal plaque and a curvature between 40 and 60 degrees, without erectile dysfunction, underwent a dermal patch alter neurovascular bound dissection. Seven patients with a 30 to 40 degree curvature underwent a plicature and albuginea' s shaving. Two patients with erectile dysfunction had an implantation with AMS CX prosthesis. Results. We obtained good aesthetic results for all patients. In 1 of the 3 patients with dermal patch, we noted minor erectile deficit treated with 20 mcg PGE1 intracavernous administration. We noted a 2 cm penis shortening in 6 patients who underwent a plicature. Conclusions. We believe that a dermal patch is indicated for patients with an elevated degree dorsal curvature without erectile disfunction. Plicature is a better and more rapid technique, which can be used in those patients who accept the shortening of the penis. We prefer prosthesis implantation in patients with erectile dysfunction.
