In this short review, we examine the most peculiar diagnostic, pathophysiological and therapeutical issues which characterize chronic heart failure (CHF) in older persons. A frequently atypical clinical presentation - possibly leading to underdiagnosis -, together with the uncertainty on the prognostic significance of and the optimal therapeutic approach to diastolic CHF, the impact of somatic and emotional comorbidities, and substantial differences between the characteristics of patients enrolled in clinical trials and those commonly encountered in the clinical practice, are all factors that may contribute to a remarkable undertreatment of CHF in older patients.

Abstract
