Heart failure (HF) is a complex clinical syndrome and numerous studies have aimed at defining the underlying pathophysiology using recent advances in the system biology approach as well as at discovering possible biomarkers to achieve a very early diagnosis of cardiac failure. An improved strategy may be represented by a global approach to HF, as suggested by several studies in which genomics, proteomics as well as transcriptomics have been used. Metabolomics may complete this -omic approach to HF and may provide a portrait of the metabolic phenomena that are involved in HF even in the early stages of the disease. Metabolomics considers the spectrum of the metabolites as a whole. The nuclear magnetic resonance (NMR) profile contains qualitative and quantitative information on the hundreds of different small molecules present in a biological sample.
