Integration of Clinical Practice Guidelines (CPGs) with clinical information gathered in Electronic Health Record (EHR) systems improves effective enactment of CPGs in the organizational workflow. We propose an approach that supports this integration by allowing a two-step dynamic adaptation of the EHR structure according to diagnostic hypotheses and CPGs recommendations so as to support clinician's compliance evaluation. Preliminary experimentation on top of an EHR system presently in use in various Cardiology clinics at the main hospital in Florence shows the feasibility of the proposed approach.
