Statin-associated autoimmune myopathy is a recently described immune-mediated necrotizing myopathy (IMNM). Unlike other statin-associated myopathies, this disease has a complex pathogenesis characterized by extensive myocyte necrosis with scarce flogistic infiltration in biopsy and anti-HMGCR antibodies. Anti-HMGCR antibody-associated necrotizing myopathy: diagnosis and treatment was illustrated using a case report. Patients with myopathy who do not improve after discontinuation of statins should be tested for anti-HMGCR antibodies and, if present, a muscle biopsy should be performed to confirm the diagnosis of statin associated IMNM, which requires aggressive immunosuppressive treatment.
