We agree that the findings presented in our study have the potential to significantly influence current guidance on hemodynamic targets for transjugular intrahepatic portosystemic shunt (TIPS). Though, to further validate our results, broad investigations, including serial assessments of hemodynamic results are warranted. Moreover, it is plausible that post-TIPS portocaval pressure gradient (PCPG) is not the only determinant of TIPS outcomes at an individual level, requiring the incorporation of additional factors such as systemic hemodynamics, comorbidities, sarcopenia-frailty, inflammatory status, and intestinal microbiota.
