Tobacco smoking is recognized as the single greatest cause of avoidable morbidity and mortality in developed countries. Notwithstanding this, physicians often forget that tobacco smoking, in addition to be a cause of many different diseases, is a disease itself and forget to make the diagnosis. Possible barriers in diagnosing tobacco dependence are here discussed. Strategies promoting diagnosis of tobacco dependence are also proposed.
