It is already more than ten years that the Italian Registry on Hb-ePTFE graft aimed to evaluate early and follow-up results of femoro-distal bypasses performed using a bioactive Hb-ePTFE graft in patients with CLI, reporting satisfactory early and long term results with the use of this graft, with comparable outcomes to autologous saphenous vein in terms of secondary patency and limb salvage rates In the present chapter, the updated results of that registry are reported, paying particular attention to the identification of those patients who could benefit from the use of such a graft even in the presence of a good quality autologous saphenous vein
